I'm running 6.2 (I should probably update). I'll try holding down the control key, but can't test as it's not so reproducible. A reboot does indeed always clear this up, so I think your last suggestion is the situation.
To answer your other question, if I tell Igor "continue waiting", the popup returns after a few seconds. If I tell Igor "stop waiting" it tries to open the given file in a pre-existing instance (if I clicked a file, otherwise it just opens a new PXP in a pre-existing instance).
Try keeping the Control key held down longer may help.
Also, if you just tell Igor to stop looking for the other Igor, do you get the multiple copies of Igor running as you want?
Lastly, there are times when the mutex Igor is looking for gets lost to the operating system, and a reboot clears this up.
